The cover is off again. It looks like the pipe itself is damaged and the cover doesn't fit right

Please note that the hole still has no cover and has ragged edges sticking up. No one has fixed this! It is a safety issue for walkers, bicycles, motorcycles etc. please address this problem. I started reporting this over a year ago. Please fix it correctly.
