Please fill defects/holes in Asphalt in front of St. Anne's Church 292 Jefferson Ave

2 holes:
-one nearer the curb 12" x 12" around metal ? gas cover
-One immediately to left of parked car 1st or 2nd space on hill) putting elders at risk to put foot into hole and to
twist ankle and fall

The concrete sidewalk along Jefferson Avenue in front of St. Anne's Rectory is elevated and is a tripping hazard. My understanding is that this was reported at the DPW by a citizen back in 2019 or so and never repaired.
In 2022 an elderly woman tripped and fell requiring ambulance transfer to the hospital as she had a fractured arm (same arm that she had a shoulder repair). I recalled her falling. She did not report it to the city but did tell me when she came by the church. (A year later), she has recovered.
Please repair this trip hazard, "Salem is for all Ages" per our City award from AARP, let's make the sidewalks safe.

Two separate areas at the entrance to Horton Street from Jefferson Ave
-Immediately on the right alongside where cars park
-2nd is along maybe where the street was dug up for water main replacement in the spring 2022; the cars and trucks riding over this area has caused the asphalt to deteriorate further revealing sand; also noise pollution as the cars/trucks are jostled about, unpleasant to hear this all day long. Drivers do not slow down.

The asphalt is deteriorating in large sections posing a safety hazard for all: elders, disabled, just about anyone crossing at the crosswalk or in the street.
This is located at the corner entryway of Story Street between St Anne's church and Rectory. The crosswalk is situated between 2 wheelchair accessible sidewalks, but would be a danger if a wheelchair or bike or foot tread upon this asphalt. This has been repaired in the past 5 years, this street is not on the list of streets for 2022 needing repaving.
Pease fill in this section before the winter thank you (travel down the street and the asphalt in front of the last 2 homes is deplorable completely devoid of asphalt in front of one driveway where children play and in front of the last home as well. The street sweeper just kicks up the asphalt & dirt, not so great for the conservation land st the end of the street nor the tires of the cars. Thank You for your attention.

1. STIRY STREET
2 city trees along the RIGHT side of the beginning of Story Street ALONG THE RED BRICK RECTORY BUILDING (St Anne's Parish Priest Residence) it is very overgrown over the rectory porch , rubbing up against the building 2-3 stories high and over the vinyl fence yard.

This was reported in ? 2018 and I was there when the city workers came out & I was told a bucket truck was needed and the city's vehicle was out of commission. I filed a report, it was closed, could someone come out and prune the tree this year before winter damage is caused to the building? One low limb is a risk for pedestrians to walk into.
2. CLEVELAND STREET right side immediately on hill, dead branch in tree waiting to fall AND tree is overhanging
St Anne's Church wall/roof. We would appreciate if the dead branch is removed and the tree trimmed up and back. Thank You

First 2 trees on right side of story street see above information

Tree on street, not in good condition, branches circling wires, street light, tree also blocks pedestrian crossing sign. On Sunday morning Dec 11th I picked up a dozen large (2-3 ft broken branches) all brttle with green lichen on them looked like I was gathering kindling , but this us Castle Hill not Little House on the Prairie.

Upon wntering Arthur Street from Jefferson Ave to the immediate right, Utility Cap missing, hole in street, safety issue; reported last year without any action

The crosswalk connecting one sidewalk handicap access (in ftont os Ste. Anne's Church) to another handicap
Sidewalk access in front of Ste. Anne's Rectory has a pothole in the middleif ....potential hazard to person in wheelchair or disabled/elderly
walking in crosswalk. Also since the street is generally in disrepair alot if silt runs down the street (both from Jeffrson Ave & Story St) ending up at the handicap entry plate to the sidewalk in front of the rectory, creating a "soft shoulder" another potentially dangerous situation if in a wheelchair. This was pointed out to the contracted supervisor last year when he was checking the work. Church volunteers are sweeping this sand /silt every couple of weeks.

Traffic turning into Champlain cut the corner
Due to it being a dangerous left hand turn
Potholder has developed, curb stone should be extended and grass restored
This results in road/asphalt debris at entry to sidewalk, dangerous for wheelchair access.

No Parking Here to Corner sign originally on a metal pile in asphalt. Years ago someone knocked it over. The sign was then secured to the telephone pole (araher high up & little further up the street) where it remained for many years though ignored by many. The sign was then vandalized & replaced this past summer 2015 by city employee who does signs ? Steve. The sign has mysteriously disappeared or ? Conveniently removed. This is an extremely busy corner & difficult to get out when cars and trucks line the corners (auto body , Tin Whistle Bar Patrons & now a flatbed truck from Bill's auto body parking not only illegally with regards to feet limitation from corner but also 109% on sidewalk!! Yes I know contact police for illegally parked cars. That could be a full-time job. Please return the sign, thank you.
