Four streetlights between Burger King and Busey Bank on the corner are stobing on and off.

Green light at NE corner is out. I reported the same light out a few weeks ago, it got fixed, and it was out again yesterday and today.

Light on the NE corner is out. The green light on the NE corner pole at the next intersection is also out.

The ground-mounted traffic light that is supposed to be visible to exiting I-74 eastbound traffic is substantially out of alignment and visible to southbound Prospect traffic. I was confused sitting on Prospect and realized I was seeing red and green lights simultaneously from different traffic lights. The WRONG light is most visible if you're where I was at the front of the southbound Prospect lane.

Note: This form requires me to pick a light color and direction that is burned out. I'm picking it because I can't submit otherwise. No light is burned out so ignore that part. The issue is that a signal head is pointing into the wrong part of traffic flow as I described above.

All streetlights are out on Town Center Blvd at least from Prospect and going as far east as I can see on both sides of the street.

Metal base of the light is detached and wiring is exposed to weather.

The cross walk sign has been hanging upside down.

There is an open hole several feet deep in the street inches from the crosswalk.

Green light on NW corner for northbound traffic has been out for a while.
