31JAN2015 I reported this same street damage, the issue was archived, and I was told by Sean S. (Street Section) that "If there has been no maintenance on your street in the next 6-9 months, please feel free to call us or re-report your issue on SeeClickFix."
It has been 9 months since I last made my report and there has not been any repair, although most of 5th St down 2 blocks south of the Lemon/5th cross streets needs some sort of repair: whether potholes, huge line cracks, or heavy street damage like the section I am reporting.
There are a lot of public funds going towards building the SolTrans Curtola Park & Ride Hub right down the street. Let's set aside some funds to fix our streets.

Thank you for responding. From the phone conversation that we had, you had stated that the work order you generated on 19OCT2015 would be fulfilled on, or around, the date of 15DEC2015.
Considering this issue has been pending since 31JAN2015, let's leave this issue open until it is actually resolved.
I will definitely be contacting you if I don't see any work started by 15DEC2015.

After contacting Sean S. today, I was told that there was a work order to have this major street damage repaired with a hot patch, that should last at least 6 months, within 7 days from now.
There is also a follow up re-surfacing that is planned to happen around JAN-FEB2016.
I will attach a few pics of the street damage currently and I will post a final pic of the hot patch when it is filled.
