I believe there's already an open service request on this, as I reported it last year and the tree was marked with orange paint. But then the ice storm came along, and I think perhaps this tree has been forgotten.

It is the middle one of three maples running in a line along St. Clair W. in Amsterdam Square Park. It's in bad condition and needs to be replaced.
